Health Benefits

Summary

This section provides details about the Group Employee Benefits Insurance plan provided by Great Eastern for Loop De Loop Pte Ltd employees. The coverage period is 15 August 2023 – 14 August 2024.

Covered Benefits

Group Hospital & Surgical Plan 6:
- Daily Room & Board: 4 Bed (Govt), max 120 days (inclusive of ICU).
- ICU Coverage: Up to SGD 10,000.
- Surgical Fees, Pre & Post Hospital Treatment (within 90 days), Emergency Outpatient & Dental Treatment, Home Nursing (up to 26 weeks).
- Miscarriage coverage (including ectopic pregnancy).
- Outpatient Kidney Dialysis (max per policy year): SGD 15,000.
- Outpatient Cancer Treatment (max per policy year): 150% of covered benefits.
- Mental Wellness Coverage: Up to SGD 5,000 per year.

Claim Process

Emergency Admission (A&E):
- Request hospital to file an e-file claim from your personal shield plan.
- Contact Great Eastern’s 24-hour Medical Concierge Services for pre-authorization e-LOG.
Scheduled Admission:
- Contact Great Eastern’s 24-hour Medical Concierge Services with details of condition, length of stay, eligibility, and estimated fees.
- Once approved, the e-LOG will be transmitted directly to the hospital.
Outpatient Treatment:
- Claims can be made via the GE Great Benefits App.
- Follow-up visits to specialists are allowed if within 1 year of previous consultation and for the same condition.
Claim Processing Time:
- Inpatient Claims: 21 business days.
- Non-panel Outpatient Claims: 14 business days.
Exclusions:
- Vaccinations, health screenings, eye examinations, cosmetic treatments, and pregnancy complications are not covered.

Mental Health Support

Summary

This section provides access to resources and support for maintaining mental wellness.

Annual Coverage Limit: Up to SGD 5,000 per year for mental health-related services.
Available resources include counseling, therapy sessions, and mental health consultation.

Claim Process

Claims can be made via the GE Great Benefits App.
